Mission & Goals
HSNY Community Health Programs provides community health services that support the agency’s broader mission and vision of ending homelessness by providing comprehensive services towards permanent housing. CHP programs also help stabilize communities and prevent homelessness for individuals at risk.
Key Programs & Initiatives
Food and Nutrition Services Program (Ryan White Part A)
This program empowers clients with HIV in managing their health through nutrition. It offers education, resources, and services that build skills and encourage healthy eating and independence around health. Services include Nutrition Counseling, Nutrition Classes, Client-Choice Pantry Services, and Light-Touch Case Management. Real Connections Program (Ryan White Part B)
This program helps clients protect, improve, and maintain their health by addressing social determinants of health such as socioeconomic factors, health literacy, food access, physical environment, and access to healthcare. Key services include Non-Medical Case Coordination, Client-Choice Pantry Services, Food Vouchers, Health Classes, and Support Groups. Eligibility: (1) HIV-positive, (2) NYS resident, and (3)

Community Pantry Program
This program helps clients address food insecurity. Key services include client-choice supermarket-style pantry services, SNAP outreach, and access to additional resources. Available by appointment via the Plentiful app for all residents of zip codes starting with 104. Click here to learn more about the Community Pantry Program
Emergency Pantry Services for HSNY Shelters
This program addresses emergency food needs of HSNY family shelters. Shelter staff choose from 10 meal-kit themed pantry bags, each with ingredients for balanced meals, recipe ideas, and nutrition education. Looking Ahead
Since 2020, HSNY has expanded its community health programming, starting with the Ryan White Part A Food and Nutrition Services Program and growing to include three additional programs funded through nine different grants. Looking ahead, HSNY plans to continue expanding its health services portfolio to further its mission of ending homelessness and providing essential support to at-risk communities.
